Easy Pass form for Civilian Access for Tournament (Easy Pass)

DEADLINE: (August 7, 2024). Civilians, please provide the following information so that we can get you processed to access the base for the tournament.  All civilian guests/golfers must enter at the San Luis Rey Gate (Vandergrift Far Right Hand Lane) where you will show your driver’s license for entrance. Please make sure the name submitted for base access is the same as your driver’s license or you will be turned away at the gate. We need the following information in the exact order: (Last Name, First Name, Middle Initial, Suffix, State of Issue, Drivers License #, Date of Birth)
